You won’t need a distinction. The vast majority of firms will be ok with a commendation - if all firms asked for a distinction, they wouldn’t be able to hire people - there aren’t enough people who secure a distinction to fill the number of TCs available.
It doesn't have to be a deal as such, it could be advisory or contentious work too.
I'd try to link it to the work that the firm would be doing - that could be linked to a general update/trend in the market, but I think it needs a clear tie to the work the firm does for it to work.

The reference request will state who the reference is on behalf of even if it is via an outsourced company.

You would be entitled to get another student visa whilst studying the PGDL, so you wouldn't have to worry about returning to India if you did go on to study a recognised course.

I would focus on the job of a junior lawyer - whether that be a trainee or NQ. You can also talk about the long term aspects of the career too, but it definitely needs balance and therefore you need to cover why your happy doing the job(s) that will get you to partner 10-15 years down the line.
It can be either or (or neither) in all honesty. It really depends on the firms in question and also how you explain things. You are rarely just asked where else you have applied to for the sake of getting names - it is much more about getting the rationale of why you have chosen those firms.
