I suspect where the LPC isn't a requirement (you could be taking the SQE route), that is why it isn't there. It is highly likely you will need to have passed either the SQE or the LPC before starting the TC, but it is worthwhile contacting the firm to check.
Firms are very risk adverse too to these things and won't take any risks in case they are fined or their sponsorship licence is taken away from them.

The "to date" to me only seems appropriate given there are 8-9 months ahead of starting and many people will get results between now and September.
Although it is possible to start a TC ahead of getting your results, most firms will expect you to have your results ahead of starting. You should speak to your LPC provider as to when the results will be published and the firm to see when their start date is.
This is not entirely true. Some assessments can factor in time taken to complete the assessment and that can then influence your percentile score, because your time taken is compared to the norm groups time taken. Therefore you can increase your percentile score by doing the test quicker than...
Sometimes it can impact your score. It really depends on how the scoring is designed (there can be lots of different factors that contribute to this though - not just timings).
There will be many firms that don’t factor in timings into their scoring system though.

The problem with the code is that it is completely voluntary - many firms don’t even stick to it, so it’s pretty difficult to ask candidates to do the same.
