When I applied, I think there was a separate section for mitigating circumstances. I assume it is a space to add anything else you think is relevant, but it may be worth contacting Grad to confirm.
For the vacation scheme it is: application form, video interview and assessment centre. I remember reading that on Chambers when I was applying too and it is incorrect.

Law firms recruit two years in advance usually, unless there happens to be a space in an earlier intake. What you are thinking of is the amount of extra time required after graduating. A law graduate has to do the LPC (1 year) and non-law students have to do both the GDL and LPC (2 years each)...
At open days, campus events etc. I've met some international students who are now trainee solicitors. They have often been at larger City firms, presumably because those firms are readily willing to sponsor their visa.
This is more of a practical point, but does anyone have any advice for where most vac schemers in London decide to stay during their vac scheme, in terms of accommodation etc.? Are student halls the best choice? Thank you!
I agree with you, it's very true, and I know how brutal the process is, as someone going through it right now. It is possible that there could be some recurring themes, but equally, it could be a matter of bad luck as you mentioned.
What feedback did you get from your interviews? If you are making the same fundamental errors or having the same issues with interviews, you may find the same problems with your direct TC applications. Just a thought, although it is hard to tell without knowing the context or the feedback you...
And also, I do not wish to be overly critical or anything, because I found the last livestream very useful, but is there any possibility of improving the camera and audio quality for future livestreams?
