A start date will be the employment date when your training/work experience starts. For firms recruiting 2-3 years in advance, this typically means you will do the SQE before your employment starts.

Where firms have a 2.1 requirement, they will typically be looking for a grade average of 13 or higher for it to be a 2.1 equivalent or higher.
If a firm has a 2.2 requirement, then it will be 11 or higher.
Annoyingly, it depends on the firm and the method of submitting the cover letter. Generally, I'd say these days you don't need to include addresses, but there are some that full letters formalities will be required.
