Honestly do not worry - we are all human and have awkward things like this happen! The firm will understand and it may have subconsciously helped you loosen up for the rest of the VI :)
@freya___ how many firms have you applied to? The fact you are a medical student should not work against you, but most candidates will be primarily facing rejections regardless of their degree - so my first piece of advice is to apply to as many firms as possible.

If it is simply that the task is difficult then I would say it is always better to first take initiative. If there is something that involves signing off on important documents for a client, then you should probably not do so without consulting the associate/partner in charge. Hope that helps :)
