I very much doubt it would be everyone who applies attends. If it’s an in-person event, an insight day can sometimes be more selective that vacation schemes or TCs (depending on the criteria for the programme and also when it is running).
I’d go for something like:
Third year (60% average): Module title 1 - 50%; Module title 2 - 70%; Module title 3 - 60%; Module title 4 - 60%
Second year….(same format as above)
First year….(same format as above)

You can still claim them, it’s just the firm might not take them into consideration.
It’s odd language to use as some mitigating circumstances don’t impact your studies. Instead they can impact your ability to build out work experience or extra curriculars, and there would be no “exam board” to...
Sounds like a good story to me. A question like this is really looking for your analysis and opinion rather than reciting facts, but obviously you can use evidence to back up your analysis.

The university is the exam board, but if they are not aware of the circumstances, they aren’t going to be able to provide anything even if they could meet the timescales.
Universities can’t apply mitigating circumstances retrospectively. It is why raising it with the university is so important...
Yes - a good place for that kind of detail. You don't need to tell them what you did though - they know what it consists of, so just focus on 1 or 2 very brief points of what you took away from completing it.
